Sursum Corda is a home located at 4605 Fairhills Road East, in a quiet residential neighborhood in Minnetonka. It is not a wing of a large facility. Set on 1½ wooded acres, the walk-out rambler is intentionally small, with space for up to 10 residents to live, gather, and receive highly personalized dementia care in a true home setting.
Gianna Homes has been caring for older adults and families since 1999. Today, we offer residential memory care in Minnetonka and Plymouth, along with in-home care throughout the western Twin Cities.

Dementia changes over time, and care needs often become more complex. At Gianna Homes, residents can remain in the home they know as those needs change, including through advanced dementia and the end of life.
Our care team works closely with families, physicians, nurse practitioners, hospice providers, and other care partners to support comfort, dignity, and changing medical needs. For families, that continuity can mean fewer disruptive transitions and the reassurance of knowing their loved one can continue to be cared for by familiar people in a familiar place.

Even as dementia progresses, there are still opportunities for connection, comfort, routine, music, conversation, favorite foods, time outdoors, and familiar activities. Our team looks for ways to meet each resident where they are and create meaningful moments throughout the day.
